We proudly serve patients and families in private homes, assisted living facilities, skilled nursing facilities, hospitals, and our care center. We serve 22 counties throughout Tennessee, Georgia and North Carolina. Our home office is in Chattanooga, TN.

We are currently seeking a Nurse Practitioner to join our group of dedicated and experienced professionals.

The Nurse Practitioner is responsible for all medical and clinical hospice care services (home visits, consults, orders, prescriptions and documentation) directed through the Hospice Medical Director. Provides medical support as needed for teams and team members.


Graduate of an accredited school for a Master’s Degree in Nursing and Nurse Practitioner Licensure.

Minimum of five years in nursing, two years in home health and/or hospice.

Nurse Practitioner licensed in the State of Georgia

Must maintain accreditation standards as they relate to licensure and Hospice policy at all times. Any deviation of such requirements requires a temporary suspension until in compliance.


Responsible for hospice care consults under the supervision of the Medical Director.

Coordinates all medical services with patient’s primary physician and hospice medical director in regard to hospice care consults and pain control measures.

Initiates orders and prescriptions as needed and documents same within 24 hours.

All visits require appropriate documentation/charting.

All changes in plan of care must follow the hospice plan of care, if a hospice patient.

All changes in medicine, etc. must be documented and communicated to the case management nurse.

Provides feed back to the referring physician for any hospital admissions and/or discharges as needed.

Ensures availability of physician services for routine and emergency situations and provides back-up as requested by the Hospice Medical Director.

Acts as liaison between the attending physician, Hospice Medical Director and the IDT as needed.

Ensures conflicting patient care decisions are resolved in a timely manner, involving the Team Leader and/or Chief Nursing Officer at all times and communicating same to the Hospice Medical Director.

The Nurse Practitioner will provide dietary education to the patients/families as necessary.

Makes appropriate visits as ordered and/or required.

Responsible for professional services to any HOC patients referred for services irrespective of their ability to pay for such services.

Recommends and monitors compliance with approval clinical guidelines in the care of patients in the HOC program.

Responsible for writing narcotic prescriptions for Hospice patients and reviewing medications with the Pharmacy.

Responsible for maintenance of script pads at all times. Any missing, questionable and/or altered scripts should be reported to the Hospice Medical Director and/or VP- Corporate Compliance immediately.

Assures face-to-face documentation is accurate format and completed in the scheduled time frame.

Responsible for administrative duties related to Hospice:

Responsible for or participates in developing and revising written patient care objectives and procedures.

Serves as a member on disciplinary team as needed.

Works with HOC staff to evaluate patient care by retrospective and concurrent audit.

Works to facilitate problem solving activities regarding Hospice services in conjunction with community physicians, other HOC departments, and community organizations.

Works to assure the efficient and cost-effective utilization of HOC resources.

Assists the Medical Director, Chief Nursing Officer and Team leaders as needed on special projects with supervisor approval.

Assists in Quality Assurance.

Participates in comprehensive quality assurance activities for the purpose of documenting, analyzing, and evaluating the quality of care as requested.

Works to assure compliance with all applicable local, state, and federal standards, laws, and regulations.

Ensures consistency of services, quality of care, and safety of patients.

Assists and/or promotes a positive Hospice environment.

Consistently evaluates work environment, seeking self-improvement, including better time-management, and more efficient work methods as observed by the director.

Represents the director and Hospice staff appropriately to customers.

Contributes to the smooth and effective operation of Hospice.

Works cooperatively with co-workers.

Demonstrates a working knowledge of Hospice policies, demonstrating ability to assess a situation, consider the alternatives, and choose appropriate courses of actions.

Always inspires confidence in customers and co-workers by being well-informed and prepared.

Demonstrates ability to organize, prioritize, and determine work needs.

Maintains strict confidentiality.

Willingly accepts additional work assignments or projects as indicated by timely and acceptable completion of such projects.

Completes annual skills competency assessment.
